About The Position

The New Providence Women’s Shelter is a 130 bed 24/7 emergency shelter for homeless women diagnosed with mental illness and/or substance use disorders. Our comprehensive services include case management, individual and group counseling, recreational activities and housing placement assistance. Additionally, an on-site Article 28 Clinic provides primary and behavioral health care services.

Requirements

  • High School/GED Diploma and experience as described below or Associate's Degree
  • One to two years (1-2) year of experience working with homeless adults, substance use and/or mental health populations.
  • Teamwork, Organizational, Interpersonal, Communication, Computer, Bilingual a plus
  • Ability to work cooperatively with peers and other staff in order to serve the needs of the clients as effectively and efficiently as possible.
  • An ability to work independently and to adjust one’s schedule to accommodate the needs of the clients.
  • Excellent oral, writing, and listening skills.
  • An ability to interface with clients as well as all levels of staff.
  • Knowledge of Case Management software (CARES).
  • Proficiency in Microsoft Office Suite, especially Outlook, Word, and Excel.
  • Bilingual or multilingual a plus.

Responsibilities

  • Conducts intake and assessments, makes necessary referrals for services and completes/updates psycho-social assessments of clients.
  • Provides case management services and assists clients in the creation of and adherence to their Independent Living Plans by identifying short and long-term objectives of which can be achieved.
  • Inputs all client information and updates in a timely fashion into the CARES system as well as the client’s physical chart.
  • Works collaboratively with clients and other staff to identify and obtain financial entitlements, housing and other concrete services
  • Maintains accurate and up to date written client documentation as required and in accordance with City/State oversight agency guidelines
  • Provides guidance for clients through individualized session on issues such as achieving personal goals, finding permanent or alternative housing, maintaining sobriety, complying with medication protocols and pursuing employment opportunities.
  • Maintains liaison with representatives from community organizations.
  • Provides escort services to clients’ appointments as needed.
  • Attends and participates in the program meetings.
  • Performs other duties as assigned by the Team Leader, Clinical Director or Program Director.
