About The Position

The New Providence Women’s Shelter is a 130 bed 24/7 emergency shelter for homeless women diagnosed with mental illness and/or substance use disorders. Our comprehensive services include case management, individual and group counseling, recreational activities and housing placement assistance. Additionally, an on-site Article 28 Clinic provides primary and behavioral health care services.

Requirements

High School/GED Diploma and experience as described below or Associate's Degree
One to two years (1-2) year of experience working with homeless adults, substance use and/or mental health populations.
Teamwork, Organizational, Interpersonal, Communication, Computer, Bilingual a plus
Ability to work cooperatively with peers and other staff in order to serve the needs of the clients as effectively and efficiently as possible.
An ability to work independently and to adjust one’s schedule to accommodate the needs of the clients.
Excellent oral, writing, and listening skills.
An ability to interface with clients as well as all levels of staff.
Knowledge of Case Management software (CARES).
Proficiency in Microsoft Office Suite, especially Outlook, Word, and Excel.
Bilingual or multilingual a plus.

Responsibilities

Conducts intake and assessments, makes necessary referrals for services and completes/updates psycho-social assessments of clients.
Provides case management services and assists clients in the creation of and adherence to their Independent Living Plans by identifying short and long-term objectives of which can be achieved.
Inputs all client information and updates in a timely fashion into the CARES system as well as the client’s physical chart.
Works collaboratively with clients and other staff to identify and obtain financial entitlements, housing and other concrete services
Maintains accurate and up to date written client documentation as required and in accordance with City/State oversight agency guidelines
Provides guidance for clients through individualized session on issues such as achieving personal goals, finding permanent or alternative housing, maintaining sobriety, complying with medication protocols and pursuing employment opportunities.
Maintains liaison with representatives from community organizations.
Provides escort services to clients’ appointments as needed.
Attends and participates in the program meetings.
Performs other duties as assigned by the Team Leader, Clinical Director or Program Director.
