The primary responsibility of the Case Manager is to initiate, organize, plan, develop, and implement direct case management and advocacy services to assigned participants and their families as part of the Merrimack County Intensive Service Option (ISO) In-Home program. Coordinate services and resources for participants, including social, educational, and medical support Advocate for participant's needs and ensure they are receiving appropriate services. Maintain accurate and up-to-date case records and documentation. Documentation is in accordance with New Hampshire Medicaid Standards. Collaborate with healthcare providers, social workers, therapists, and other professionals All service plans are based on strength-based approach using the wrap around model. Provide crisis intervention and support as needed. Applicant is dependable, committed and able to serve as a positive role model for youth in community, school, and home settings.
