About The Position

The primary responsibility of the Case Manager is to initiate, organize, plan, develop, and implement direct case management and advocacy services to assigned participants and their families as part of the Merrimack County Intensive Service Option (ISO) In-Home program. Coordinate services and resources for participants, including social, educational, and medical support Advocate for participant's needs and ensure they are receiving appropriate services. Maintain accurate and up-to-date case records and documentation. Documentation is in accordance with New Hampshire Medicaid Standards. Collaborate with healthcare providers, social workers, therapists, and other professionals All service plans are based on strength-based approach using the wrap around model. Provide crisis intervention and support as needed. Applicant is dependable, committed and able to serve as a positive role model for youth in community, school, and home settings. The position offers flexible hours, competitive weekly pay and activity reimbursement.

Requirements

  • Bachelor’s degree in social work, psychology, education or a related field with an emphasis in human services
  • At least two years of experience working with children and families
  • Knowledge of community resources and services in Merrimack County and New Hampshire.
  • Experience in the provision of home and community-based services.
  • Demonstrated written and verbal communication skills
  • Proficiency in computer skills including Microsoft Office Suite.
  • Reliable transportation, valid driver’s license, and current automobile insurance coverage is required.

Nice To Haves

  • Experience using Electronic Health Record (EHR) or case management software a plus.
  • Bilingual/Spanish speaking applicants are a plus
