About The Position

Under the direction of the Program Manager, the Drop -In Center Case Manager provides high -quality, center -based case management services to individuals and families accessing services through the Drop -In Center. The Case Manager serves as a primary point of contact for participants, supporting stabilization, service linkage, and long -term self -sufficiency through structured, on -site support services.

Requirements

  • Knowledge of homelessness, poverty, and social service systems
  • Strong interpersonal, organizational, and communication skills
  • Ability to work with diverse populations with complex needs
  • Experience conducting intakes, assessments, and service planning
  • Familiarity with Los Angeles County social services and community resources
  • Trauma -informed, culturally responsive, and client -centered approach to care
  • Ability to work in a fast -paced, high -volume service environment
  • Flexible schedule, including some evenings and weekends as needed

Nice To Haves

  • Experience with case management documentation systems preferred

Responsibilities

  • Conduct client intakes and comprehensive needs assessments at the Drop -In Center
  • Complete psychosocial assessments and develop individualized service plans in collaboration with participants
  • Provide on -site case management services in a welcoming, trauma -informed environment
  • Assess client needs and connect participants to appropriate community resources and referrals
  • Provide resource navigation and service coordination, including housing referrals, healthcare access, benefits enrollment, employment resources, and social services
  • Maintain accurate and timely documentation, case notes, and client records
  • Build trusting, professional relationships with participants
  • Support clients in achieving stability goals related to housing, health, income, and self -sufficiency
  • Coordinate care with internal staff and external service providers
  • Utilize harm reduction, strengths -based, and client -centered approaches
  • Manage an active caseload of participants receiving Drop -In Center services
