About The Position

The Case Manager works collaboratively with Program Manager to provide comprehensive case management services to clients of WestCare diagnosed with substance use disorders, mental health conditions, or co-occurring disorders.

Willing to seek out new work projects and contribute ideas and efforts to the constant improvement of WestCare services.
Previous case management experience in substance abuse and/or mental health required.
High School diploma or equivalent required.

Nice To Haves

Bachelor’s degree in human services field preferred.

Responsibilities

Educate and refer clients to community agencies and businesses that appropriately address the clients' needs for food, clothing, shelter, employment transportation, etc.
Utilize and manage Electronic Health Record (EHR) systems for data entry and client information tracking, ensuring program alignment.
Assist clients with establishing or re-establishing support systems within their community.
Act as advocate on behalf of the client to improve access to the services needed.
Participate in clinical staffing decisions regarding the client’s current strengths and resources and their need for additional resources.
Complete documentation to services on provided forms in a timely manner.
Provide input into the client’s treatment needs and disposition.
Assist other WestCare staff as needed in a collaborative manner to promote cooperation and portray a professional image to the community.
Embrace and embody mission, vision, guiding principles, clinical vision and goals of WestCare Foundation.
Perform other duties, as assigned.
