About The Position

The Doe Fund is a leading New York City non-profit organization that operates the enormously visible Ready, Willing & Able (RWA) program, whose participants clean over 115 miles of streets and sidewalks every day. With an $80 million budget, 500+ staff members, and 46,000 individual donors, The Doe Fund is nationally acclaimed as a work-based model for solving homelessness and criminal recidivism. We empower people to break the cycles of homelessness, welfare dependency, substance abuse, and incarceration through innovative work and housing programs. We are seeking a Case Manager for The Doe Fund's newest permanent supportive housing for homeless individuals living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. This 98-unit residence will provide housing for 49 individuals referred by the New York City HIV/AIDS Services Administration (HASA), and the remaining 49 units will be apartments for moderate-income households. The Case Manager is responsible for providing supportive case management services to the residents. The successful candidate will have strong clinical and communication skills, along with experience working with the HIV/AIDS population, and will be available to cover some evening hours.

Requirements

Bachelor’s Degree in Social Work or relevant field
Minimum of three years of post-bachelor experience
Ideal candidate must have a working knowledge of HIV/AIDS, substance abuse, mental illness, and homelessness
Excellent case management skills, including knowledge of benefits in the NYC Metro area
Sensitivity to community issues and needs
Outstanding interpersonal and communication skills, both oral and written
Strong ethical standards, confidentiality, and personal integrity
Flexibility with schedule to cover evening activities
Computer proficiency (i.e., email, Microsoft Word, Excel, and PowerPoint)

Responsibilities

Maintain a caseload and provide supportive case management services to these individuals
Maintain resident records in compliance with agency and funding standards
Conduct and review assessments
Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency
Monitor and ensure residents are linked to healthcare providers
Provide counseling and crisis intervention as necessary
Assist residents in securing and maintaining benefits
Maintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programs
Facilitate groups and activities that promote health and independent living skills development
Monitor and assist residents with rent payments and budgeting
Conduct regular unit inspections
Educate residents about approaches to mental health
Attend ongoing training to enhance the services provided to the residents
Assist with the completion of reports as needed
Other duties as assigned
