Case Manager - Adult Mental Health Services (Mid-Intensity)
About The Position
The Case Manager for Adult Mental Health Services, provides case management and clinical services for individuals with mental illness, substance disorders use and/or developmental disabilities and their families. Services include supportive counseling, individual and family support, crisis intervention, assessment, Individualized Service Plan (ISP) development, psycho-education, and referrals to clients and their families. Develops and maintains working relationships with: individuals served, families, additional service providers, and other community agencies as needed to ensure quality services. Works as part of a treatment team to ensure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community.
Requirements
- Bachelor's Degree is required in psychology, counseling, social work or special education.
- Registration as a QMHP is required. Must maintain QMHP status and all required trainings/CEUs.
- At least one year of experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities is required.
