About The Position

Gateways Hospital and Mental Health Center is seeking a motivated individual to fulfill the role of Case Manager in the Statewide Transitional Residential Program (STRP). The STRP is a 35-bed (24 male, 11 female), unlocked, residential, forensic program, and is the highest level of care with the CONREP system (CAs Conditional Release Program for individuals transitioning from the Department of State Hospitals back into the community). Clients are referred from county CONREP programs throughout the state and the STRP program serves as a transitional, 90-day, step-down program between the state hospital and the community. SUMMARY OF POSITION: As part of the interdisciplinary STRP treatment team, the Case Manager is responsible for guiding clients in the re-integration into the community through the establishment of benefits (Social Security, Veterans Administration, Medi-Cal, ACCESS, etc.) and assistance in accessing outside medical, dental, financial, housing, transportation, clothing, retail, and immigration resources. The Case Manager assists clients in developing skills and accessing resources to support their community integration, and collaborates with the treatment team and residential staff to ensure quality care for all clients.

Requirements

  • Prior case management experience.
  • Academic training in Behavioral Sciences.
  • Experience with the mentally ill and/or forensic populations.
  • Must pass Department of Justice (DOJ), Federal Bureau of Investigations (FBI), and Community Care Licensing (CCL) background clearance.
  • Valid California Drivers license
  • Driving record acceptable for coverage by Gateways insurance carrier
  • Minimum Education Required Bachelors Degree in Behavioral Sciences or related area.

  • Desired Education Masters Degree in Behavioral Sciences or related area.

Responsibilities

  • Responsible for securing all benefits for clients through liaison with Social Security, Medi-Cal, Veteran’s Administration or other benefits agency.
  • Keeps the Program Coordinator informed about clients’ benefit status.
  • Assists clients in obtaining disabled bus passes, California I.D., immigration documentation, and other services as needed.
  • Coordinates with the RN and FNP to arrange for assisting clients in accessing medical, vision, and dental care.
  • Transports and accompanies clients to and from appointments as determined necessary.
  • Guides and assists clients in making purchases in the community and on-line.
  • Assists clients in connecting with supports in the community via video.
  • Assists with discharge preparation by taking clients to visit their next program or living situation.
  • Understands documentation requirements for case management services.
  • Documents all services provided within program guidelines and expectations in a timely manner.
  • Provides the treatment team with ongoing updates on clients’ functioning, appointments, outcome of appointments, and required interventions.
  • Completes and documents all services rendered within expected time frames.
  • Assists in situational crisis intervention as needed.
  • Reports safety hazards, maintenance and housekeeping needs to the Residential Manager.
  • Performs other additional tasks as directed.
