The Case Manager role involves coordinating admissions, providing transportation, facilitating access to housing resources, and developing discharge plans to support successful community reintegration. In accordance with RNP's Mission Statement, Philosophy of Client Care and Code of Ethics as stated in the Personnel Policies and under the supervision of the Clinical Coordinator, manage client care for individuals requiring multiple assistance sources by performing the following duties: HOURS - Monday- Thursday, Sunday- 8:00am- 4:00pm
