Under the supervision of the Minor Shelter Program Manager, the Minor Case Manager will be responsible for providing ongoing engagement, assessment, intervention, and evaluation for all youth ages 12- 17. This includes youth intakes, assessments, crisis phone counseling, client data collection, documentation, file maintenance, as well as follow-up and aftercare. This position also involves regular communication and mediation with the youth’s family, if applicable, and working towards positive program termination within 21 days of entry. In addition, the Minor Case Manager will assist with general upkeep of the Minor Shelter program, including program shift coverage, milieu management, crisis intervention, reporting maintenance issues, and ensuring that the program is facilitated in a manner that meets Urban Peak’s mission. The Minor Case Manager will work with youth ages 12-27 who are enrolled in Urban Peak’s Minor shelter program who are experiencing homelessness. The acuity of clients varies based on mental and physical health needs, and unique histories of trauma. The priority for these clients will be family reunification or, if not possible, referring to other systems of care within 21 days of program entry. This position will ensure that interactions, interventions, and environments at Urban Peak are trauma informed and will emphasize the physical and emotional safety of youth and staff.
