About The Position

Join The Doe Fund on our mission to break the cycles of homelessness, addiction, and recidivism. Our flagship Ready, Willing & Able (RWA) program provides paid transitional work experience, transitional housing, advanced occupational training, and comprehensive supportive services to individuals experiencing homelessness. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent, affordable, and supportive housing, with more than 1,900 units in operation or pre-development. The Doe Fund has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs. We are seeking a Case Manager for one of our supportive housing programs. The Case Manager is responsible for providing supportive case management services to Peter Jay Sharp residents, including assistance with obtaining public benefits, monitoring residents’ rent payments, developing independent living skills and socialization skills, and facilitating groups and recreational activities.

Requirements

  • You have a Bachelor's degree in Social Work or a relevant field.
  • You have at least 3 years of experience working with homeless individuals diagnosed with serious mental illness and/or substance abuse disorder and/or in trauma settings
  • You have expertise in Evidence-Based Practice, including Critical Time Intervention, Motivational Interviewing, and Harm Reduction techniques
  • You have excellent clinical and case management skill,s including knowledge of benefits in the NYC Metro area
  • You are sensitive to community issues and needs
  • You have outstanding interpersonal and communication skills, both oral and written
  • You have strong ethical standards, confidentiality, and personal integrity
  • You are computer proficient (i.e., email, Microsoft Word, Excel, and PowerPoint)

Responsibilities

  • Maintain a caseload of individuals diagnosed with serious mental illness and/or substance use disorder and provide supportive case management services to them
  • Develop and facilitate therapeutic groups for residents
  • Maintain resident records in compliance with agency and funding standards
  • Conduct and review assessments
  • Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency
  • Monitor and ensure residents are linked to appropriate treatment and services
  • Provide counseling and crisis intervention as necessary
  • Assist residents in securing and maintaining benefits
  • Maintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programs
  • Monitor and assist residents with rent payments and budgeting
  • Conduct regular unit inspections
  • Educate residents about approaches to mental health and substance use
  • Compile resources on mental health information and treatment strategies
  • Attend ongoing training to enhance the services provided to the residents
  • Assist with the completion of reports as needed
  • Other duties as assigned
