About The Position

Brave Space Alliance (BSA) is a Black- and Trans-founded LGBTQ+ focused community center on Chicago's South Side that welcomes and serves all who come through our doors with affirming, culturally competent resources, programming, and services. We strive to empower, embolden, and educate each other through mutual aid, knowledge-sharing, and the creation of community-sourced resources as we build a world of liberty and justice for all. The Case Manager plays a critical role in building trust within communities historically harmed by systems of care. This position goes beyond service linkage; it centers dignity, safety, and self-determination for LGBTQ+ individuals navigating housing instability, economic hardship, health inequities, and systemic barriers. This role requires strong relational skills, the ability to remain grounded in high-acuity environments, and a deep commitment to culturally responsive, community-based support.

Requirements

  • Proficiency in Microsoft Office and case management/data systems.
  • Bachelor’s degree in Social Work, Human Services, or related field — or equivalent lived experience and community-rooted leadership.
  • 2–3 years of experience in case management or direct service work.
  • Familiarity with trauma-informed care and harm reduction frameworks.
  • Experience working with LGBTQ+ and BIPOC communities.

  • Knowledge of housing systems, public benefits, and social service navigation.
  • Experience supporting individuals experiencing homelessness or housing instability.
  • Bilingual (English/Spanish).

Responsibilities

  • Conduct comprehensive intake assessments and develop individualized service plans.
  • Maintain an active caseload of approximately 25–40 clients (or as assigned).
  • Provide referrals and warm handoffs to housing, healthcare, behavioral health, public benefits, and other essential services.
  • Advocate for clients to ensure access to affirming, culturally competent, and trauma-informed providers.
  • Support clients navigating systemic barriers, including homelessness, re-entry, public benefits systems, and criminal legal involvement.
  • Provides responsive, cross-functional support to program staff to ensure continuity of services and team effectiveness.
  • Maintain accurate, timely, and confidential documentation in compliance with organizational and legal standards (within 48 hours of client contact).
  • Monitor client progress and adjust service plans as needed.
  • Conduct consistent follow-ups to ensure continuity of care and stability.
  • Maintain professional boundaries while working within a community-centered space.
  • Respond to urgent client needs including housing displacement, interpersonal violence, and mental health crises.
  • Utilize trauma-informed, harm reduction, and de-escalation practices.
  • Remain regulated and grounded while supporting individuals in crisis.
  • Understand and adhere to mandated reporting requirements where applicable.
  • Represent BSA at outreach events and community initiatives as needed.
  • Build and maintain relationships with affirming service providers and community partners.
  • Track service delivery, client outcomes, and program metrics aligned with grant and organizational requirements.
  • Enter and maintain data in designated case management systems.
  • Contribute to impact reporting by documenting measurable outcomes and client narratives while maintaining confidentiality
  • Attend weekly supervision meetings with Behavioral Health Manager, bi-weekly Programs Team meetings, and monthly all-staff meetings.
  • Other duties, as assigned.
