The Case Manager provides case management support services to residents living in permanent supportive housing for Single Adults and Families formerly experiencing homelessness. The Case Manager provides case management to his/her 1:25 ratio caseload and is responsible for service connection, information and referral, crisis intervention, community organizing, groups and activities focused on community building, education, skill development, and housing retention. Services are provided within the context of harm reduction practices and a Housing First model.
