About The Position

America Works of New Jerseyis seeking a compassionate, organized, and results-driven Case Manager to provide comprehensive pre- and post-employment case management services. This role supports individuals transitioning toward sustainable employment and self-sufficiency. The Case Manager works closely with job developers, trainers, county case managers, and community partners to ensure participants receive the support, resources, and guidance necessary to succeed. This role requires strong documentation skills, professionalism, and the ability to manage multiple clients at various stages of program engagement.

Requirements

  • Bachelor's degree preferred (Social Work, Human Services, or related field).
  • 1-3 years of case management or workforce development experience.
  • Strong organizational and time-management skills.
  • Proficiency in Microsoft Office; CRM experience preferred.
  • Ability to work with individuals experiencing significant life challenges.
  • Strong written and verbal communication skills.

Responsibilities

  • Conduct intake interviews and job readiness assessments.
  • Develop individualized employment service plans.
  • Provide pre- and post-employment support, including retention follow-up.
  • Identify and address barriers to employment (housing, childcare, transportation, healthcare, clothing).
  • Assign participants to appropriate community work experience sites based on skills and location and follow up with attendance sheets.
  • Mentor clients transitioning from public assistance to employment.
  • Coordinate with county case managers and government agencies.
  • Collaborate with community partners such as The One Stop Training Center and Goodwill Helms Academy.
  • Research and develop new resource partnerships.
  • Provide weekly updates to shared caseworkers and counselors.
  • Maintain detailed case notes, attendance, and progress reports.
  • Track participant progress in CRM and state systems.
  • Submit required weekly and monthly reports (GA timesheets, Engage by Cell forms, DNR reports).
  • Document success stories and program outcomes.
  • Ensure compliance with county and program requirements.
  • Provide conflict resolution and de-escalation support.
  • Model professionalism and inclusivity.
  • Meet weekly, quarterly, and annual performance metrics.
