About The Position

We are seeking a compassionate and experienced Case Manager to support clients in achieving self-sufficiency. Working as part of an interdisciplinary team, the Case Manager will develop individualized case plans, provide critical support, and connect clients to resources that address housing, mental health, recovery, and employment needs.

Requirements

  • High school diploma required; Bachelor’s degree in a Human Services discipline (e.g., Psychology, Social Work, Criminal Justice) strongly preferred.
  • At least 5 years of relevant experience with a focus on mental health, addiction, aging, trauma, or related areas.
  • Bachelor’s degree in Social Work must include LSWA-level licensure.
  • Strong knowledge of D.C. social service resources and compliance requirements.
  • Demonstrated cultural competence with the ability to effectively communicate and build relationships with a diverse population.
  • Strong interpersonal skills, including conflict resolution, problem-solving, and professional boundary management.
  • Excellent organizational skills and the ability to multi-task in a fast-paced environment.
  • Proficiency in Microsoft Word and Excel, with a willingness to learn new database systems (knowledge of HMIS is a plus).
  • Self-motivated and capable of working independently.
  • Friendly, professional demeanor with a commitment to maintaining confidentiality.

Responsibilities

  • Foster a safe and supportive environment for clients.
  • Collaborate with the Lead Case Manager to address client challenges related to housing, finances, employment, and mental health.
  • Co-facilitate client groups alongside the Lead Case Manager when appropriate.
  • Conduct bi-weekly face-to-face case management meetings, with one meeting in-house, ensuring compliance with organizational standards.
  • Communicate regularly with the Lead Case Manager or Program Manager regarding client progress, successes, and challenges.
  • Partner with external agencies and professionals to coordinate services and preferred treatment methods.
  • Participate in organization-wide meetings, team training, and departmental events.
  • Present clients’ psychosocial needs and strengths during interdisciplinary and care coordination meetings.
  • Maintain thorough and accurate case documentation, including assessments, referrals, legal documentation, and service updates.
  • Report serious incidents or grievances immediately to the Program Manager and complete incident reports as required.
