About The Position

The Case Manager serves as a primary point of contact for clients enrolled in our food delivery programs. In this role, you will conduct outbound and inbound calls to screen, assess, and schedule food deliveries for clients in need. You will work within a HIPAA-regulated environment where accuracy, confidentiality, and integrity are essential — all actions are logged and subject to audit. This role requires strong bilingual communication skills in both English and Spanish, with a preference for Spanish-dominant candidates who are also proficient in English.

Requirements

  • Bilingual in English and Spanish required; Spanish-language proficiency strongly preferred.
  • Prior experience in a call center, case management, social services, or client-facing administrative role preferred.
  • Ability to conduct structured phone screenings and document findings accurately.
  • Comfortable reviewing eligibility documents and applying program criteria consistently.
  • Proficiency in Microsoft Excel at a basic level (data entry, tracking, scheduling).
  • Ability to navigate web-based portals, enter data, and leave clear and accurate case notes.
  • Strong attention to detail and commitment to accuracy — errors in this role have real impact on client services and program compliance.
  • High level of integrity and professionalism; understanding that all actions within program systems are logged and auditable.
  • Excellent verbal and written communication skills in both English and Spanish.
  • Strong organizational and time management skills with the ability to manage a high volume of client contacts.

Responsibilities

  • Contact clients by phone to conduct screenings and assess eligibility for food delivery services based on pre-existing screening criteria.
  • Review and evaluate client screening documents to confirm eligibility and ensure all required information is complete and accurate.
  • Schedule food deliveries across multiple program formats, coordinating with clients to confirm availability and delivery preferences.
  • Accurately enter and maintain scheduling and tracking information in Microsoft Excel spreadsheets.
  • Navigate and operate an internal web portal to record client interactions, make program selections, and leave detailed case notes in a clear and timely manner.
  • Maintain strict confidentiality and adhere to all HIPAA regulations governing client information; all actions may be subject to future audit.
  • Communicate professionally and compassionately with clients in both English and Spanish to ensure clear understanding of program benefits and next steps.
  • Flag any discrepancies or eligibility concerns to a supervisor for review.
  • Perform other duties as assigned.
