About The Position

Mountain View Hospital is looking for a Case Manager to join our team! JOB SUMMARY: This position provides support for the LCSW. And requires a passionate commitment to core values and behaviors. Responsibilities- Coordinates and assists with new patient set up and resource contact for LCSW. Functions as a Case Manager for the Cancer Patients in conjunction with the LCSW. Requirements

  • Education/Certifications: Bachelor’s Degree in social work, nursing, psychology, or health administration or equivalent experience.
  • Must have a working knowledge of the Microsoft Office suite products (Word, Excel, & Outlook).
  • Knowledge of basic Workers Compensation and medical terminology is essential.
  • Must have a current driver’s license.
  • Knowledge of Medical practice, and Medical Terminology.

Nice To Haves

  • Preferred C-SWCM (certified social work case manager)

Responsibilities

  • Helping LCSW in assessing clients’ physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plan.
  • Facilitates flow of information regarding patients’ course among care providers.
  • Keeps strict confidentiality with patients’ information and follows all HIPAA laws
  • Listening to clients’ concerns and providing resources or referrals as required.
  • Following up with patients’ progress periodically, collaborating with the LCSW and making adjustments as needed to improve outcomes.
  • Keeps a spreadsheet of patients who have been referred out for counseling.
  • Helps schedule patients to be seen with the LCSW for counseling.
  • Scans/Copies and uploads documents into EMR Aria and Cerner.
  • Helps medical staff provide resources to patients.
  • Maintains client contact list. And updates folders as needed.
  • Monitors supply inventory and orders supplies appropriately.
  • Adheres to the Hospital policies and demonstrates safe practices.
  • Demonstrates competence in the Environment of Care to provide functional and safe surroundings for patients and staff (i.e. safety, security, infection control, etc.) Able to locate applicable hospital and department specific manuals.
