Job Title: Case Manager Division/Program: (Children/Youth Inpatient) Psychiatric Health Facility Starting Compensation: 22.00 - 28.00 USD Per Hour Working Location: Bakersfield, CA Working Hours/Shift: Monday - Friday 8 am to 4:30 pm Why Join Our Team? Associate Degree required Bachelor’s degree in a social science or related field preferred. (2) years’ experience in a mental health setting Preferred Bilingual in Spanish preferred Experience with case management services preferred. How you will make a difference: Participates in initial assessments and obtains outpatient treatment provider information, options, and resources. Verifies follow-up appointments and collaborates with follow up treatment and step-down services. Conducts groups to assist patients with transition to outpatient/community-based services. Collaborates with PHF staff and caregivers to plan for and schedule discharge. Provide community resources and psychoeducation. Submits discharge packets to referred agencies. Documents all services provided in a clear and concise manner within the Electronic Medical Record within the timelines and framework established. Transport clients to and from appointments, hospital visits, etc. as needed. Develops and maintains relationships with external partners, including DBH and DSS and outpatient providers. Requirements

  • High school diploma or equivalent required.
  • Associate Degree required
  • (2) years’ experience in a mental health setting

Nice To Haves

  • Bachelor’s degree in a social science or related field preferred.
  • Bilingual in Spanish preferred
  • Experience with case management services preferred.
