About The Position

The substance use case manager will provide comprehensive case management services to individuals with substance use disorders. This position is for the evening expanded hours program, evenings are required. This role will focus on assessing client needs, developing individualized service plans, coordinating treatment, and support services. This will help promote long-term recovery. The case manager will work collaboratively with the senior counselor, client, families, treatment providers, and community resources to ensure continuity of care and support harm reduction, recovery oriented goals. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.

Requirements

Bachelor's degree in related field required.
CADC preferred.
Valid Driver's License required.
Annual, American Heart Association BLS and Heart Saver First Aid required within 90 days of hire or transfer. CPI required within 90 days of hire or transfer.
Ability to pass a background check through fingerprint process and drug screen as required.
Previous case management experience required.
Strong interpersonal and communication skills
Ability to build a rapport with client and maintain professional boundaries
Ability to problem solve and manage crisis
Organizational skills and attention to detail
Ability to work with a diverse population
Needs to work both independently and as part of a team

Responsibilities

Conduct initial screenings and ongoing needs
Develop, implement, and monitor individualized treatment and recovery service
Provides crisis intervention, individual counseling, and strength based
Facilitate referrals for mental health, housing, employment, court, and other supportive services.
Monitor client progress, provide follow-up, and adjust service plans as
Coordinate or provide transportation for clients when
Maintain accurate timely confidential documentation in compliance with HIPPAA state regulations and AtlantiCare addiction services policies.
Ensure compliance with grant requirements, program standards, and funding
