Family Service League is seeking a full-time Case Manager for a homeless shelter in Smithtown, NY. The Case Manager will be responsible for conducting client assessments, developing Independent Living Plans, and linking clients with community-based service resources. The Case Manager will assist clients in achieving self-sufficiency through advocacy, counseling, and resource referrals. Family Service League, established in 1926, is a social services agency transforming lives and communities through mental health, addiction, housing, and essential human services with more than 60 social service programs in over 20 locations.
