We are currently seeking a Case Manager for our Children Youth and Family Services department in Buffalo, New York. The Case Manager (CM) will provide services to families, including referrals, counseling support, and crisis. The Case Manager will provide services to families, including referrals, counseling support, and crisis for each family. The CM will maintain relationships with referring partners and reentry providers, as well as provide case management services to the participating families. This position will be responsible for monitoring a caseload and will report to the Program Manager.
