The successful candidate will be expected to: Work as part of a team to provide case management to individuals receiving services in the community through OPWDD funding (Self Direction program). Support individuals with disabilities to utilize their services and funds to live in the community. Communicate (primarily via phone, email or zoom) with the individuals and their support team, assisting them to utilize their budget according to OPWDD rules and regulations. Obtain and maintain required documentation. Review all documentation for accuracy and guidelines adherence. Document all communications.
