About The Position

The Case Manager is responsible for individuals in SVRC Vocational and Community Living Support programs, providing support based on their plan of service. This role involves managing vocational rehabilitation program participants, authoring designated plans of service, implementing and coordinating the plan, writing progress reviews, and communicating with referral contacts and guardians. The Case Manager also handles participant intake and orientation, coaches individuals on various work-related topics, and supports them in the community.

Requirements

  • Effective written and oral communications skills.
  • Proficient in Microsoft Office products.
  • Ability to work with a diverse population.
  • Ability to display reasoning; decision-making skills, and empathy.
  • Successful completion of Criminal History Background Check and Drug Screen is required under the terms of agency employment.
  • Safe driving record (for drivers).
  • Able to secure a valid State of MI Chauffer's License (for drivers).

  • Bachelor's degree in the human services field or other applicable area.

Responsibilities

  • Responsible for vocational rehabilitation program participants, authoring designated plans of service, implementing and/or coordinating the implementation of the plan, writing progress reviews of the plan, and communicating with the designated referral contact person and/or guardians about the plan and progress.
  • Responsible for special assignments within Vocational and Community Living Support programs, as assigned by supervisor.
  • Completes intake and orientation of participants to program or for services to be provided; may include tours, conveying of rules and expectations, signing necessary documents, conducting initial interview, and eliciting participant's cooperation and interest in the program.
  • Implements person-centered plan, addendums, progress reviews at prescribed intervals.
  • Communicates (written and verbal) with referral contact person (and parent/guardian, if applicable) concerning participant's progress and/or problem development in a timely manner; informs supervisor of critical incidents immediately.
  • Coaches participants on various work-related topics (e.g. appropriate grooming and dress, job seeking skills, work behaviors, how to impress a supervisor, behaviors which may lead to dismissal, potential jobs in the community, overcoming barriers, etc.).
  • Supports program participants in the community through follow along services.
  • Transports participants as needed for program participation.
  • Communicates with job coaches or others on how to work with individual participants.
  • Maintains documentation in accordance with agency, CARF, and Community Mental Health standards and requirements.
  • Represent the clinic at community events, task forces, or committees as assigned.
  • Support agency initiatives by participating in pilot projects or newly developed services.
  • Attend all required administrative and team meetings.
  • Promote a safe and secure environment through awareness, reporting, and use of safety protocols.
  • All other duties as assigned.
