About The Position

Since 1905, The Bradley Center has continually served children and families in need. Today, the residential treatment facility provides 24 hour care and trauma-informed treatment to children who have a history of trauma and who may present with a wide range of challenges.
We are seeking a Case Manager to provide quality therapeutic care management services for children and their families as part of a residential treatment team.

Requirements

• Bachelor’s degree in social work, counseling or other related field
• Minimum of one year experience working in a human services field (CASSP system-related field preferred)
• Must be at least 21 years of age
• Valid driver’s license and car insurance
• Organization and strong attention to detail
• Excellent verbal and written communication skills
• Professional demeanor, sound judgment, dependability, and a strong work ethic
• Problem-solving skills

Responsibilities

• Schedule and facilitate Interagency team meetings
• Coordinate treatment services and discharge planning
• Conduct chart audits
• Communicate with families and external sources effectively and in a professional demeanor
• Complete required documentation of tasks completed, complete monthly status updates and treatment plan updates
