About The Position

Health Solutions West is the Western slope’s largest behavioral health care organization, covering more than 23,000 square miles across 10 counties. With over 250 employees in Western Colorado, you would be joining a mighty team of support and administrative staff, case managers, clinicians, physicians, nurses, and others in our efforts to improve the health and wellbeing of our community. This Case Manager role is an embedded position within the Oasis Clubhouse that supports both clubhouse operations and the delivery of comprehensive case management services. The Oasis Clubhouse is a peer-run drop-in center offering community support and empowerment. This role serves as a bridge between informal peer engagement and formal behavioral health services by building relationships with members and supporting their transition into ongoing care, treatment, and community supports. This position utilizes the Oasis Clubhouse as a primary access point and engagement hub, creating a pathway for members to receive coordinated case management, behavioral health services, and recovery supports. The Case Manager works collaboratively with peer staff, clinical teams, and community partners to ensure continuity of care, improved outcomes, and sustained engagement.

Requirements

  • Candidates must hold a bachelor’s degree in human services or a related field.
  • A bachelor’s degree in a non-related field may be considered if accompanied by at least one year of experience in a healthcare or human services setting.
  • A minimum of two years of experience in mental health, substance use, or community-based case management is preferred.
  • Applicants must possess a current, valid Colorado Driver’s License and have reliable transportation.
  • Candidates should demonstrate strong engagement, organizational, and interpersonal skills and have the ability to work effectively in a peer-centered, recovery-oriented environment.
  • Working knowledge of Windows based computer applications to include Microsoft Word, Microsoft Excel, Microsoft Outlook, and Internet access.
  • Ability to operate standard office equipment including calculator, copy machine, fax and multi-line telephone.
  • Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers.
  • Ability to organize and prioritize workload in a sometimes hectic environment with frequent interruptions.
  • Knowledge of mental illness, substance abuse, and psychopathology and associated treatment modalities
  • Working knowledge of Colorado Revised Statutes 27-10 (Mental Health Code)
  • Working knowledge of HIPPA and 42 CFR.
  • Knowledge of community health and mental health resources and their eligibility criteria
  • Ability to work in a cooperative and collaborative manner within a clinical treatment team setting.
  • Ability to follow and implement direction as provided by the Clinical Team and/or any Supervisor who may be designated by the Clinical Supervisor or Program Coordinator.
  • A working knowledge of individual, group, family, and crisis treatment approaches and their application to patients with emotional and behavioral problems.
  • Must already be authorized to work in the US; sponsorships not available.

Nice To Haves

  • Previous experience working with individuals with co-occurring mental health and substance use conditions is strongly preferred.
  • Certification as an Addiction Counselor II or III is strongly preferred.

Responsibilities

  • Provide on-site case management services to Oasis members.
  • Conduct assessments, develop individualized treatment plans, and coordinate care.
  • Assist members with accessing behavioral health services, medical care, housing, benefits, employment, and other community resources.
  • Facilitate warm handoffs to internal programs and external providers.
  • Support members with appointment scheduling, follow-up, and care navigation.
  • Monitor progress toward goals and adjust service plans as needed.
  • Contribute to the development of best practices for clubhouse-based case management.
