Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency in Arizona, providing residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. The Case Manager provides case management services in either the community and/or in office setting to patients and their families. This role involves assisting patients and the care team in planning, connecting, outreaching, advocating, coordinating, and monitoring patient needs and progress. Case Managers help patients access health services, financial assistance, employment, education, social services, and natural supports. They are responsible for assessing patient needs, developing and monitoring individualized treatment plans, and accurately documenting services. The position emphasizes embracing the recovery philosophy and promoting a safe recovery environment for clients.
