About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency in Arizona, providing residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is a non-profit provider of substance use and behavioral health programs, utilizing cutting-edge, nationally recognized, evidence-based models. The Case Manager provides case management services in either the community and/or in office setting to patients and their families. This role involves assisting patients and the care team in planning, connecting, outreaching, advocating, coordinating, and monitoring patient needs and progress. Case Managers help patients access health services, financial assistance, employment, education, social services, and natural supports. They are responsible for assessing patient needs, developing and monitoring individualized treatment plans, and accurately documenting services. The position emphasizes embracing the recovery philosophy and promoting a safe recovery environment for clients.

Requirements

  • Highschool diploma or GED required
  • 1-3 years of experience working with the GMH/SU population required
  • 1-3 years of experience in behavioral health position providing outreach and engagement activities required
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
  • Valid Identification required
  • Arizona Fingerprint Clearance Card

Nice To Haves

  • Associates degree of higher in a field related to behavioral health preferred
  • 1-3 years of experience in a medical setting preferred
  • Minimum of 6 months of recovery from substance use and/or mental health disorders preferred
  • Current AZ Driver’s License (valid and in good standing) preferred
  • Clean 39-month driving record preferred

Responsibilities

  • Provides case management services in either the community and/or in office setting to patients and their families

  • Assesses patient needs
