About The Position

The Case Manager is a highly organized, empathetic, compassionate, nonjudgmental, and eager to help and who functions as the lead case coordinator. The case manager is responsible for working alongside a client to help them identify and meet personal goals, while adhering to the milestones and expectations of the program at large. Using trauma-informed and strengths-based approaches, the case manager will help the client navigate the requirements of the program, identify client’s goals and objectives, and ultimately support their journey to self-sustainability.

Requirements

  • Social Work or Psychology Bachelor’s degree plus 1 year of experience; OR AA degree plus 2 years of experience working with individuals or families with complex needs.
  • Works independently, flexibly, and as part of a team.
  • Works effectively with outside agencies in providing client services.

Nice To Haves

  • Fluency in Spanish language (spoken and written)
  • Trauma informed training
  • Bi-lingual/bi-cultural knowledge is desirable.
  • Preferred education emphasis in human services, counseling, guidance, social work, etc.

Responsibilities

  • Conduct thorough assessment of client and family needs and strengths.
  • Develop, implement, and review individualized service plans with client.
  • Work collaboratively with client and with other community resources to meet/achieve client needs and identified goals and objectives.
  • Demonstrate proficient skills and abilities regarding verbal and written communication as well as conflict management and mediation.
  • Maintain accurate and current progress notes for all clients on caseload.
  • Attending all staff meetings as required.
  • Provide case management to clients and their families, including behavior management techniques.
  • Facilitate psychoeducation classes and groups.
  • On-duty assignments providing crisis intervention, resources, and referrals.
  • Complete treatment plans, assessments, progress notes, monthly reports, exit paperwork, and other required paperwork in a timely manner.
  • Maintain close communication with all treatment team members.
  • Participate in weekly individual and/or group supervision as required.
  • Attend and participate in all training courses as assigned.
  • Maintain professional behavior, appropriately representing Saint John’s mission and vision.
  • All other duties assigned by supervisor or other management.
  • Regular, predictable attendance is required.
  • Ability to get along and work effectively with others.
