About The Position

The Case Manager is responsible for providing comprehensive case management services, conducting educational lectures, and supporting clinical staff with various tasks. This role involves maintaining accurate case records, coordinating access to necessary services, and assisting with aftercare planning and referrals. The Case Manager also actively participates in clinical reviews, staff meetings, and professional development activities.

Requirements

  • A bachelor's degree in social work or an equivalent comparable human services field is required
  • Experience in case management preferred
  • Experience with mental health and/or substance abuse populations preferred
  • Ability to determine needs and to work with multi-problem individuals
  • Ability to develop rapport, trust and confidence in client relationships
  • Must possess good communication and teamwork skills
  • Strong organizational skills
  • TB Test and police clearance required
  • Authorization to work within The United States
  • Excellent computer skills required
  • Complete Safe From Harm training, and keep current as needed
  • Must possess MCBAP credential (CADC, CADC-M, CAADC) or have an approved Development Plan within 30 days of hire date
  • Valid Michigan Chauffeur License and approved by The Salvation Army MVR

Nice To Haves

  • Experience in case management preferred

Responsibilities

  • Conducts didactics/educational lectures
  • Advises clients individually or in group on issues related to risk prevention, diagnostic screenings, and community resources that improve social determinants of health outcomes
  • Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement.
  • Assist clients in determining the steps needed to take in order to gain access to treatment services and support the client with identifying and adequately addressing barriers
  • Provide clinical services to screen, assess, and determine appropriate level of care
  • Assess social needs of program participants and provide resources to the extent possible for the client to utilize when in the community
  • Assist clients in locating, joining, and finding means to participate in self-help/support groups such as NA, AA and Celebrate Recovery.
  • Maintain case records in accordance with Harbor Light standards
  • Coordinate and facilitate access to healthcare, social services, housing, financial assistance, and other support systems
  • Assist with aftercare planning and referrals
  • Participates in the review of all clinical issues, activities and program policies and procedures
  • Attends all clinical staff meetings, and team conferences
  • Participates in educational courses, seminars, workshops, current literature and in-service meetings to enhance treatment skills
  • Establish and maintain professional and timely documentation in the electronic medical records system. The case manager is responsible for his/her own documentation and is expected to review, correct, and seek assistance to ensure compliance.
  • Maintain a working knowledge of current trends and developments in substance use disorder and co-occurring disorders by reading relevant materials and participating in continuing education seminars, webinars and other in-service training when offered.
  • Perform other duties as assigned
