Case Manager, Rehabilitation Description Title: Case Manager Job Type: Full time Your experience matters! It’s designed to help individuals recover from serious injuries or illnesses that have significantly impacted their ability to function independently. Specialized Rehabilitation Programs : Tailored care for patients recovering from stroke, brain injury, spinal cord injury, amputation, orthopedic trauma, and other neurological or complex conditions. Intensive Therapy Model : Patients receive at least three hours of physical, occupational, and/or speech therapy per day, five days a week, under the supervision of a rehabilitation physician. State-of-the-Art Facility : Offers modern therapy equipment and private rooms to support a comfortable and focused recovery environment. The Licensed Case Manager is responsible for coordinating the interdisciplinary care plan, discharge planning, and patient progress communication within the inpatient rehabilitation setting. This role serves as a liaison among patients, caregivers, and the healthcare team to ensure timely and effective care transitions. Requirements

• Licensure required as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
• Minimum of 2 years of experience in social work or case management in an inpatient setting preferred
• Effective oral and written communication in English; additional languages preferred.
• Proficiency in Microsoft Office applications required.
• Strong communication and coordination skills required.
• Ability to manage complex cases and work collaboratively with medical and non-medical staff.

Nice To Haves

• Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
• acute rehabilitation hospital experience strongly preferred.
