About The Position

Catholic Charities of the Archdiocese of Newark is one of New Jersey’s largest non-profit social service agencies with programs and facilities serving Bergen, Essex, Hudson, and Union counties. We are currently seeking a Full Time Case Manager for our St. Lucy’s Emergency Homeless Shelter , located in Jersey City, NJ. St. Lucy's is an 80 bed emergency shelter in Jersey City that ensures safety and builds stability in the lives of homeless men and women.

Requirements

Bachelor's Degree required (Bachelor’s degree in social work or related field preferred)
2-3 years experience working with the homeless population or related experience required

Responsibilities

overall responsibility for delivering social services, intake, assessments, admissions, discharge
develop & implement Service Plans
homeless prevention services and related documentation and progress reports
Develop and maintain compliance with HUD, County and DCA essential services standards and related policy and procedures
Maintain referrals and communication with County Coordinated Assessment Agency
In partnership with the family develop a Housing & Service Plan and provide assistant and linkages in obtaining needed services and achieving clients goals
