About The Position

Requirements

  • Bachelor’s Degree required
  • Understanding of housing first model
  • Understanding of severe and persistent mental illness
  • Understanding of harm reduction and trauma informed care
  • Strong interpersonal skills
  • Strong written and verbal skills
  • Team player
  • Proficient in Microsoft Outlook and Excel
  • Able to use or learn computer databases such as AWARDS, MAVEN, Paycom
  • Walking and traveling to escort clients to various appointments throughout the city
  • Regular home visits to client living spaces required

Nice To Haves

  • Experience working with homeless adults preferred
  • Experience working with individuals diagnosed with severe and persistent mental illness, substance abuse and/or co-occurring disorders preferred
  • Prior case management experience preferred
  • Bilingual (English and Spanish) preferred
  • Occasional visits to hospital emergency rooms

Responsibilities

  • Serve as case manager for formerly homeless adults with mental health, substance use and other co-occurring disorders
  • Perform intake, assessment, service planning, advocacy, referral and escort services
  • Provide case management; identify short and long-term treatment goals and help clients achieve them, including linkages to community programs such as medical, psychiatric and drug treatment services.
  • Utilize evidence-based practices such as person- centered planning and motivational interviewing.
  • Maintain case records with documentation of services provided and outcomes, according to specific program procedures. Responsible for case notes and service plans using an electronic database
  • Assist clients in receiving and maintaining entitlements
  • Track and Monitor Covid cases, and Hospital visits
  • Accompany clients on visits to doctors, entitlement offices or other community services as needed
  • Serve as liaison between other service providers and community resources
  • Encourage engagement and participation in services and activities
  • Participate in weekly staff meetings, case conferences, training programs and supervision meetings
  • Assist with the design, planning, and implementation of group activities and events
