About The Position

The Case Manager is responsible for supporting individuals through intake, and program participation. This role ensures participants meet program admission criteria, receive needed services, and are supported in navigating barriers such as housing instability, legal obligations, and access to basic needs. The Case Manager serves as an advocate and connector, working closely with clients and external partners to ensure comprehensive, wraparound support.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (Master’s preferred).
  • At least 1–2 years of experience in case management, social services, or related work.
  • Strong understanding of trauma-informed care, harm reduction, and systems of support for underserved populations.
  • Ability to work independently and as part of a multidisciplinary team.
  • Valid driver’s license and reliable transportation may be required, depending on service location.

Responsibilities

  • Conduct intake assessments, develop individualized service plans, and maintain accurate case documentation.
  • Provide case management and advocacy throughout the client’s engagement with the program, including post-program follow up.
  • Identify and connect clients with appropriate partner agencies and community resources related to housing, employment, legal support, food access, mental health, and other supportive services.
  • Meet with assigned participants at least monthly to address needs, review progress, and provide encouragement and guidance.
  • Provide clients with educational materials, safety planning, and referrals to promote independence and well-being.
  • Perform ongoing follow-up to ensure services have been received and advocate on behalf of clients when services are denied or delayed.
  • Maintain current knowledge of available community resources, services, and social service best practices.
  • Submit all required documentation and reports in a timely and accurate manner.
  • Collaborate with internal staff and external stakeholders (e.g., probation officers, clinicians, housing specialists) to ensure coordinated, client-centered care.
