About The Position

The Healthy Start Case Manager is responsible for delivering comprehensive case management services to eligible prenatal, postpartum, and parenting families enrolled in Hamilton Health Center’s Healthy Start program. Healthy Start serves pregnant women and families with children up to 18 months of age who reside in Dauphin County. The Case Manager works to improve access to medical care and community resources, promote healthy behaviors before, during, and after pregnancy, and support safe and healthy infant and toddler care practices through a patient-centered, community-based approach.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Sociology, or a related field, preferred.
  • Candidates with a bachelor’s degree in a non-related field, or a combination of relevant education and experience related to the functions of the position, will be considered.
  • Prior CPR and/or First Aid certification required or ability to obtain upon hire.
  • Valid driver’s license with the ability to travel throughout the service area.
  • Experience performing case management or comparable social service functions, preferred.

Nice To Haves

  • Minimum of two (2) years of experience in case management or a comparable social services role.
  • Prior experience working with pregnant, postpartum, or parenting families in a community-based setting.
  • Knowledge of Joint Commission regulations.
  • Familiarity with community resources in Dauphin County and surrounding areas.
  • Bilingual in English and Spanish.

Responsibilities

  • Conduct home, community-based, or office visits with assigned participants every two weeks or monthly, in accordance with program guidelines.
  • Perform case management activities including participant screenings and assessments using Healthy Start data collection tools.
  • Develop, implement, and monitor individualized service care plans.
  • Connect participants to appropriate community resources and facilitate and coordinate timely referrals.
  • Track participant health information and outcomes.
  • Conduct risk evaluations and advocate for clients as needed.
  • Provide emotional support while maintaining professional boundaries and ethical conduct.
  • Deliver evidence-based education related to pregnancy, postpartum care, child development, risk reduction, and healthy behaviors.
  • Ensure compliance with all documentation, contact requirements, and reporting standards required by funding organizations and regulatory agencies.
  • Enter all required documentation accurately and timely into the designated electronic database.
  • Attend all required department and team meetings.
  • Complete and maintain all continuing education and training required by program regulations.
