About The Position

Guild is hiring Case Managers to join our Targeted Case Management teams serving adults primarily in Dakota County and Ramsey County. This position is responsible for assessing needs, identifying goals, and developing a plan for services with individuals served who are diagnosed with serious and persistent mental illness, substance use disorders, and co-occurring medical conditions. This includes referring and linking the individual to needed services and resources, as well as monitoring progress and coordinating ongoing services.

Requirements

  • Knowledge of severe and persistent mental illness (SPMI) and the needs of people with SPMI.
  • Ability to identify and assess a wide range of client needs.
  • Knowledge of local community resources and eligibility requirements.
  • Skills in a variety of treatment approaches including person-centered planning, harm reduction, and motivational interviewing.
  • Ability to develop and maintain receptive, open, and approachable lines of communication with clients and staff, and provide a friendly environment for visitors.
  • Strong written and verbal communication skills, including composing, proofreading, and editing.
  • Presentation and meeting facilitation skills.
  • Ability to work as a proactive and positive member of a team to deliver quality services.
  • Ability to maintain confidentiality of organization, personnel information, and client data.
  • Ability to use sound judgment and maintain healthy professional boundaries.
  • Analytical and problem-solving skills.
  • Ability to meet deadlines and set priorities.
  • Proficiency with a variety of web-based software and databases, including Microsoft Office Suite.
  • Bachelor's degree in one of the behavioral sciences (social work, psychology, or human services) or a related field from an accredited college or university AND have 2,000 hours (1 year) of experience in the delivery of service to persons with mental illness OR
  • Qualified as a mental health practitioner within the State of Minnesota by one of the following:
  • At least 30 semester/45 quarter credits AND 2,000 hours of supervised experience in the provision of mental health services. OR
  • 4,000 hours of supervised experience in the provision of mental health services.
  • Must not be actively receiving services from Guild and must not have received services within the last two years.
  • A valid Class ‘D’ Driver’s License, reliable vehicle, satisfactory driving record and necessary insurance.
  • Current CPR/First Aid Certification is required.
  • Ability to pass a MN Department of Human Services background check.

Nice To Haves

  • Demonstrated experience providing Targeted Case Management services.
  • Demonstrated experience working with individuals who are unhoused and/or experience engaging in outreach efforts.
  • Previous professional experience connecting clients with federal, state, and county programs.

Responsibilities

  • Establish therapeutic engagement with individuals served.
  • In collaboration with individuals served, assess needs, identify goals, and develop a plan for services with a recovery focus.
  • Identify useful services and resources and connect individuals as needed.
  • Coordinate ongoing services and monitor progress. Provide continuous assessment and input in the planning of service levels that meet individuals’ needs.
  • Engage in crisis intervention practices-- allowing for individuals to be safe in their home and community.
  • Complete all required documentation in a timely and accurate manner.
  • Creatively problem solve, explore new alternatives, and actively engage individuals in solutions that support stabilizing mental health while maintaining a positive working relationship.
  • Establish and maintain ongoing, positive relationships with other staff members, community service providers, property managers, and families.
