About The Position
RiseWell Community Services is seeking a full-time Case Manager to join our Affordable Residential Housing Team in Brooklyn, NY In this role, you will provide case management services, housing support and assistance to individual’s age 55+, who have chronic health conditions and need assistance with activities of daily living. Assist residents with overcoming barriers associated with their health conditions so they may live in on-site in their apartments successfully. Provide information, training, hands on assistance and skill building to individuals served.
Requirements
- BA in Human Services or equivalent
- Clean, valid NY State Driver’s License
- Fingerprinting, criminal record check, approval from NYS Office of Mental Health
