About The Position

The Case Manager plays a critical role in supporting participants at Genny’s Place by coordinating services, connecting individuals to community resources, and helping them achieve their recovery and housing goals. This role requires a compassionate, proactive professional who can advocate for participants, manage multiple responsibilities, and work collaboratively within a multidisciplinary team.

Requirements

Bachelor’s degree in Human Services or related field preferred (or equivalent experience)
Minimum of 2 years of experience supporting individuals in behavioral health, substance use, or IDD services
Case management experience preferred
Valid driver’s license and reliable transportation required
Intermediate computer skills and ability to use electronic systems
Strong communication and interpersonal skills
Ability to build trust and rapport with diverse populations
Knowledge of community resources and behavioral health systems
Crisis intervention and problem-solving skills
Organizational skills and attention to detail
Commitment to ethical practice, confidentiality, and person-centered care

Responsibilities

Collaborate with a multidisciplinary team to develop and implement person-centered recovery plans
Partner with participants, clinical staff, and community stakeholders to support goal achievement
Build and maintain relationships with community agencies and service providers
Provide community-based case management services focused on housing stability, recovery, and independence
Assist participants with accessing benefits, housing, and community resources
Transport participants to appointments and support engagement in services
Complete assessments, service plans, and required documentation in compliance with organizational and regulatory standards
Maintain accurate and timely records in accordance with program and funder requirements
Identify service gaps and recommend improvements to enhance program effectiveness
