About The Position

The Case Manager plays a critical role in supporting participants at Genny’s Place by coordinating services, connecting individuals to community resources, and helping them achieve their recovery and housing goals. This role requires a compassionate, proactive professional who can advocate for participants, manage multiple responsibilities, and work collaboratively within a multidisciplinary team.

Requirements

  • Bachelor’s degree in Human Services or related field preferred (or equivalent experience)
  • Minimum of 2 years of experience supporting individuals in behavioral health, substance use, or IDD services
  • Case management experience preferred
  • Valid driver’s license and reliable transportation required
  • Intermediate computer skills and ability to use electronic systems
  • Strong communication and interpersonal skills
  • Ability to build trust and rapport with diverse populations
  • Knowledge of community resources and behavioral health systems
  • Crisis intervention and problem-solving skills
  • Organizational skills and attention to detail
  • Commitment to ethical practice, confidentiality, and person-centered care

Responsibilities

  • Collaborate with a multidisciplinary team to develop and implement person-centered recovery plans
  • Partner with participants, clinical staff, and community stakeholders to support goal achievement
  • Build and maintain relationships with community agencies and service providers
  • Provide community-based case management services focused on housing stability, recovery, and independence
  • Assist participants with accessing benefits, housing, and community resources
  • Transport participants to appointments and support engagement in services
  • Complete assessments, service plans, and required documentation in compliance with organizational and regulatory standards
  • Maintain accurate and timely records in accordance with program and funder requirements
  • Identify service gaps and recommend improvements to enhance program effectiveness
