About The Position

As a Case Manager with HELP USA, you will play a crucial role in assisting individuals experiencing homelessness to secure housing and achieve self-sufficiency. Your responsibilities will include helping clients identify the root causes of their homelessness, collaborating with other members of the service team to formulate a personalized independent living plan, and conducting regular meetings with clients to support them in executing their plan and finding a home. Additionally, you will provide guidance and referrals for essential services such as employment, education, medical and mental health support, childcare, and substance abuse treatment, tailored to each client's unique circumstances.

Requirements

  • Bachelor's degree in social work or a related field
  • Are able to keep track of multiple priorities, completing tasks on time and keeping the appropriate people informed of the status
  • Great communication skills (written and oral)
  • Want to help people experiencing homelessness

Responsibilities

  • Help clients identify the causes of their homelessness
  • Coordinate with other members of the service team to develop an independent living plan
  • Meet with clients regularly to help them work their plan and find a home
  • Provide direction and referrals for employment, education, medical and mental health services, childcare, substance abuse, and more
