About The Position
The Case Manager will refer clients to community resources provided by WCGC or other appropriate service provider and will complete documentation required for the records. This includes but is not limited to development of the case management plans for POCCI, recording of community resources, inventory of items used for outreach. The Case Manager will provide case management services including but not limited to exploring and identifying community resources, facility communication between community resources and other providers, assisting persons-served in identifying and locating needed resources. The Case Manager will work alongside the Peer Support Specialist in the POCCI program. Essential Job Functions include those listed below. ESSENTIAL JOB FUNCTIONS: Builds and maintains relationships with all community partners. Will attend all required Pasco County Opioid Task Force meetings as well as internal staff meetings. Will plan, along with Peer Support Specialist, 1-2 weekly outreach events to distribute Narcan, resource info and other items required by grant. Tracking of inventory of items used by the grant as well as tracking referrals made. Maintain data according to company policy and contract requirements. Provide daily, weekly, and monthly reports as requested. Assist and monitor data collection and statistical reporting. Maintain relationships with referring and supporting agencies and represent the agency at various community meetings as requested. Coordination of various services throughout several agencies. Attend scheduled meetings and presentations with referral agencies; and Perform any other duties as assigned. Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation. During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you. Perform all other duties as assigned.
Requirements
- Excellent interpersonal and communication skills

- Understanding to talk to and communicate with clients in an effective and appropriate manner
- Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality
- Professional appearance and demeanor
- Must be able to obtain and maintain level II background clearance
- Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required
- Two (2) years’ experience working in substance abuse prevention, community health promotion, or related fields is highly desirable
- A strong understanding of substance abuse issues, mental health promotion, and evidence-based prevention strategies is essential
- Bachelor’s degree from an accredited university or college required
- Bachelor’s degree with a major in counseling, social work, psychology, nursing, rehabilitation, special education, health education, or related human serviced field is required
Nice To Haves
- Bilingual (English/Spanish) is preferred
- Familiarity with theories of behavior change, public health principles, and community organizing approaches is advantageous
