About The Position

A growing Health Center in Rockland County is seeking a dedicated Behavioral Health Case Manager to support clients in accessing and coordinating essential mental health services. This role is ideal for someone who is compassionate, organized, and committed to helping clients navigate care, develop treatment plans, and connect with appropriate resources to support their overall well-being. Community Medical & Dental Care, Inc., has over 60 health care providers who offer a wide range of services including Adult Medicine, Pediatrics, Family Practice, Obstetrics/Gynecology, Dentistry/Oral Surgery, Allergy, Dermatology, Endocrinology, Nutrition Counseling, Occupational Therapy, Ophthalmology, Optometry, Podiatry, Speech Therapy, and Urology. Requirements

  • Experience with referrals and obtaining services for clients.
  • Knowledge of relevant federal, state, and local regulations.
  • Bachelor's degree in social work, psychology, or a related field.
  • Ability to work independently and carry case load without constant monitoring.
  • Experience collaborating with a multidisciplinary team.

Responsibilities

  • Complete health screening assessment
  • Conduct comprehensive assessments of clients' behavioral health needs then
  • Develop individualized treatment plans based on assessments and client input.
  • Referral and related activities to obtain needed services.
  • Monitor client progress and adjust treatment plans as necessary.
  • Coordinate care between multiple service providers to ensure cohesive treatment.
  • Advocate for clients' access to necessary services and resources.
  • Educate clients and their families about mental health conditions and available treatments
