About The Position

This role supports individuals involved in recovery, behavioral health services, and community supervision by coordinating services, connecting participants to resources, and helping remove barriers to stability. Requirements

  • High School Diploma or GED
  • Experience working with individuals impacted by behavioral health challenges, substance use, housing instability, or justice involvement
  • Strong communication and interpersonal skills
  • Reliable transportation and valid California driver’s license
  • Ability to work collaboratively in a multidisciplinary team environment

Nice To Haves

  • Degree in Social Work, Human Services, Psychology, or related field
  • Experience in case management, recovery services, or community-based programs
  • Familiarity with trauma-informed care and recovery-oriented service models

Responsibilities

  • Conduct participant intake and orientation to program services
  • Complete comprehensive needs assessments to identify behavioral health, housing, employment, and recovery support needs
  • Develop individualized case management and recovery plans
  • Provide crisis intervention and ongoing support when needed
  • Monitor participant progress and engagement in services
  • Connect participants to services including behavioral health treatment, substance use recovery programs, housing services, employment resources, and medical care
  • Coordinate with treatment providers, behavioral health partners, probation, recovery programs, and community organizations
  • Participate in multidisciplinary meetings to support participant stability and program outcomes
  • Use trauma-informed, strengths-based, and recovery-oriented approaches when working with participants
  • Support individuals in identifying goals and building long-term stability
  • Help participants navigate barriers and access community resources
  • Maintain accurate and timely case documentation and service records
  • Enter and maintain client data in program databases including Empower, Tyler Supervision, SPARS, and GPRA systems
  • Assist with program reporting, grant compliance, and evaluation activities
  • Assist program staff with operational and administrative needs
  • Support program documentation, supplies, and participant records
  • Maintain professional relationships with community partners
