About The Position

The Case Manager is a key member of the interdisciplinary treatment team, responsible for coordinating and advocating for comprehensive services that address the biopsychosocial needs of clients in outpatient substance use and co-occurring mental health treatment. Utilizing a trauma-informed, person-centered approach, the Case Manager supports client recovery through care coordination, resource navigation, and ongoing support to address barriers to treatment engagement and retention.

Requirements

  • Bachelor’s degree in social work , Psychology, Human Services, or a related field preferred .
  • At least one year of experience working with individuals with SUD and/or co-occurring disorders .
  • Knowledge of trauma-informed care, harm reduction, and motivational interviewing principles.
  • Strong interpersonal, organizational, and advocacy skills.
  • Familiarity with state specific Medicaid documentation standards and local community resources.
  • Proficiency in EMR systems (Athena EMR experience preferred).

Nice To Haves

  • Case Management Certification or equivalent experience preferred .

Responsibilities

  • Conduct comprehensive case management assessments upon admission and at regular intervals.
  • Develop and implement individualized care plans in collaboration with the client and treatment team.
  • Coordinate access to social, financial, medical, legal, housing, and vocational resources to address social determinants of health.
  • Collaborate s with Peer Support Specialists to work together in assisting patients in addressing their social determinants of health
  • Monitor progress toward care plan goals and update plans as needed.
  • Maintain ongoing communication with internal clinical team members and external service providers to ensure continuity of care.
  • Facilitate warm hand-offs and follow-ups to support treatment retention.
  • Provide psychoeducation and support on navigating systems such as Medicaid, legal aid, child welfare, etc.
  • Advocate for clients' needs while promoting autonomy and empowerment.
  • Maintain accurate , timely documentation in accordance with Medicaid, Medicare, and CARF standards (including contact logs, service plans, progress notes).
  • Participate in interdisciplinary team meetings, treatment planning, and staff development trainings .
  • Identify and address potential risks to client safety and report concerns according to protocol.
