About The Position

Volunteers of America Northern California & Northern Nevada (VOA-NCNN) is a ministry of service with nearly 600 paid, professional employees dedicated to helping those in need rebuild their lives and reach their full potential. Founded locally in 1911, VOA-NCNN is one of the largest providers of social services in the region, operating more than 40 programs including housing, employment services, substance abuse, and recovery services to families, individuals, veterans, seniors, and youth. VOA-NCNN provides shelter or housing to over 3,500 men, women, and children every night. Their mission is to change individual lives, instill hope, increase self-worth and facilitate independence through quality housing, employment, and related supportive services. VOA-NCNN is recruiting for a full-time Case Manager position for their Elk Grove Housing Program. The Elk Grove Housing Program provides transitional housing assistance to Elk Grove residents experiencing homelessness, including single adults and families, in coordination with services provided by the City's homeless navigation team and/or other social service providers. Transitional housing is considered short-term housing designed to assist households with securing permanent housing as quickly as possible. The program operates three transitional properties and manages four permanent housing properties, three for adults and one for seniors (55+).

Requirements

  • A minimum of two (2) years social work-related education from an accredited academic institution or (3) years of experience working with the homeless population or alcohol/drug recovery programs, or a combination thereof.
  • Demonstrated ability in record keeping.
  • Good written and verbal communication skills.
  • Knowledge of problems unique to women, minorities, persons with disabilities, persons with a history of substance use, individuals with low to no income, and general unhoused population.
  • CPR and First Aid training within ninety (90) days of employment date and recertification as necessary.
  • A valid California driver’s license.
  • Ability to meet organizations insurance carrier guidelines.

  • A Bachelor’s Degree in Social Work or a related field is preferred.

Responsibilities

  • Responsible for performing daily work requirements to achieve established objectives of the department.
  • Provide records and services in compliance with contract policies, regulatory and funding source requirements.
  • Maintain positive relationships with program participants, landlords, Volunteers of America staff and all community service agencies.
  • Create and maintain case management files on each participant which will include personal information, educational and job skills, short-term and long-term plans to become self-sufficient in all areas of his/her life and documentation of participants’ progress and assistance in removing obstacles to set goals.
  • Provide evaluations of individual clients’ needs to maintain their housing.
  • Document all follow-up and placements.
  • Provide crisis management as needed and mediate conflicts with fellow participants, community partners and neighbors.
  • Complete all recertification agreements and extension requests with participants as needed.
  • Assist clients in maintaining compliance with participation agreement.
  • Complete all discharge paperwork in a timely fashion.
  • Assist clients in Temporary Housing with Intensive Housing Searches.
  • Complete budget and housing stabilization plans with participants.
  • Provide resource information and appropriate case planning specific to any participant who has physical or mental impairment.
  • Participate in Volunteers of America and outside training sessions.
  • Perform other reasonably related duties as assigned by the Program Manager.
  • Conduct monthly house meetings.
  • Meet with participants weekly.
  • Arrange or facilitate participant transportation, when needed.
  • Liaison between clients and community service agencies to meet client needs.
  • Maintain a list of all available community services and providers.
  • Assure a safe, non-judgmental environment for the clients.
  • Maintain positive, professional relationships with clients and staff.
  • Participate in agency and outside training sessions.
  • Maintain files on each participant short- and long-term plans to maintain and improve self-sufficiency.
  • Document progress and assistance in removing obstacles to set goals.
  • Provide routine documentation of coordination and follow-up of all areas and collecting financial information.
  • Maintain participant data in HMIS and Credible.
  • Provide records and services in compliance with agency policies, regulatory and funding source requirements.
  • Maintain daily, clear and thorough communication with the Program Manager on all areas of responsibility listed in this job description.
  • Document and report all client concerns and/or infractions that impact program security.
  • Secure approval for exceptions to standard policy.
  • Notify management of potential workflow problems and resource needs necessary to attain performance standards.
  • Adhere to staffing schedules to provide adequate/safe staffing coverage and to accomplish program objectives.
  • Provide emergency shift coverage as needed.
  • Perform reasonably related duties as assigned by the Program Manager.
  • Assist in department orientation and training for new employees as requested to ensure quality work outcomes.
  • Identify appropriate ongoing training for both new and existing staff and report needs to management.
  • Serve as part of the program development team by providing information and support for the development of quality operations.
  • Maintain an above average working knowledge of fire, safety, and health standards to assure safe work environment for clients and all personnel.
  • Continually learn and enhance technical and interpersonal skills.
  • Attend staff meetings/assigned training and complete required certifications, i.e. CPR, First Aid, etc.
