About The Position

The Lifeline case manager supports individuals and families in building healthy, resilient relationships through group instruction, personalized coaching, and case management. This role is ideal for someone passionate about personal development, skilled in life coaching, and committed to empowering clients through education and support services. Reports to the Program Manager.

Requirements

  • Experience in case management, coaching, or related role
  • Strong verbal and written communication and active listening skills.
  • Ability to guide discussions and create a collaborative environment.
  • Ability to adapt to different learning styles.
  • Ability to address challenges and make adjustments as needed.
  • Computer skills including Microsoft Office, PowerPoint, Google Drive, etc.
  • Agreement with Lifeline’s STATEMENT OF FAITH AND STATEMENT OF PRINCIPLE
  • Must be a committed Christian demonstrating a personal relationship with Jesus Christ and faithfully attend church
  • Ability to prepare materials, manage time, and make referrals
  • Ability to adjust facilitation methods based on individual client needs and feedback.

Nice To Haves

  • A degree in a related field (e.g., Education, Social Work) highly desired.

Responsibilities

  • Educate new clients about all of Lifeline's services
  • Foster a safe environment that encourages participation and reflection.
  • Assist in program enrollments and appointment scheduling
  • Provide individualized coaching to clients focused on relationship skills, personal growth, and goal-setting.
  • Conduct intake assessments and develop personalized action plans.
  • Connect clients with internal and external support services as needed.
  • Maintain accurate and timely documentation of client interactions, attendance, and progress.
  • Utilize digital platforms to track outcomes, manage referrals, and support grant reporting.
  • Ensure confidentiality and compliance with data protection policies.
  • Assist with outreach and recruitment efforts for program participants.
  • Collaborate with team members to evaluate program effectiveness and improve service delivery.
  • Participate in staff meetings, training sessions, and community events as a Lifeline representative.
