Requirements

  • Bachelor’s degree in education, psychology, or other human services-related field preferred and a minimum of 6 months of full time experience in human services.
  • Relevant full-time employment experience in developmental disabilities services may substitute for any portion of a bachelor’s degree at the rate of six (6) months of full-time employment experience for each semester.
  • Valid Kansas drivers license.

Responsibilities

  • Serve as the client advocate for all persons assigned to receiving case management or related services in a manner that assures the protection of rights, the maintenance of confidentiality, and ongoing client satisfaction in all issues relating to service provision.
  • Assure compliance of all client files/documentation with applicable licensing and accreditation requirements.
  • Provide and share information about services and resources available, provide referrals, and link individuals to the support and services they need.
  • Keep abreast of current developments in community-based service delivery.
  • Monitor, implement, and successfully manage state/federal funding program procedures to ensure program compliance and maximize allowable billing.
  • Coordinate, develop, and monitor person-centered plans in a manner that helps establish natural supports and community inclusion.
  • Complete or coordinate the completion of assessments as determined necessary by support needs or person-centered plans.
  • Communicate effectively with individuals served, their families, designated representatives, and support and service providers relative to the service plan and related support needs.
