Job Posting: Case Manager- EISEP Location: Rochester, NY Department: Care Coordination- Aging and Adult Services Employment Type: Full-Time OR Part-Time Schedule: Monday- Friday (40 hours) OR Monday- Friday (20 hours) Salary: $22.00/hr. Don't miss out on being a part of something special! General Description Under direction provides case management services to functionally impaired older adults. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.

Requirements

  • Bachelor’s degree in human services or related field OR Registered Nurse with one year experience in the field of Aging. (BSW preferred). Equivalent combination of education and experience will be considered.
  • Valid NYS Driver’s license with reliable transportation.
  • 2 years in Human Services, preferably in the field of Aging.

Nice To Haves

  • Bilingual in Spanish preferred.
  • New York State online Case Manager Certificate preferred or willingness to complete training course within first four months of hire.

Responsibilities

  • Provides case management services to functionally impaired elderly.
  • Screens individuals for program eligibility.
  • Conducts in-home psychosocial assessments to determine client need and financial status with client and caregiver.
  • Completes full assessments on computer.
  • Develop and implement comprehensive care plans.
  • Authorizes and arranges for home care through contracted Aide Agencies/Day Programs.
  • Authorizes for payment of bills by tracking services.
  • Monitors clients and services in accordance with state regulations.
  • Develops and implements appropriate discharge plans.
  • Keeps up-to-date records and documents according to policy.
  • Acquires knowledge of EISEP regulations and standards.
  • Keeps updated on new regulations and standards set by State Office for Aging.
  • Acquires up-to-date knowledge of aging services and professional practices through reading and attending seminars, workshops and conferences.
  • Participate in staff meetings and case reviews.
  • Complete all statistics and write reports in a thorough and timely fashion.
  • Conducts outreach and public relation activities as requested.
  • Participates in relevant Agency meetings and/or trainings
  • Actively pursues development of professional competencies related to job performance through reading, supervision, in-service training and attendance at conferences and workshops.
  • Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse
  • Other duties as assigned
