The Case Manager Social Worker provides therapeutic intervention and social work services to patients and their families to enhance comprehensive, integrated, and uninterrupted care in the hospital and to facilitate continuity of care in the community. Demonstrates positive interpersonal communication with patients, families, visitors, and co-workers in accordance with organization and departmental policies and procedures. GENERAL SUMMARY: The Case Manager is a member of the hospital or specialty-based patient-centered care team or treatment team responsible for the collaborative practices of identification, assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual's and family's health care needs. The goals of the case manager are to promote patient safety, quality of care and cost-effective outcomes. The case manager provides effective discharge planning services that support a safe transition to the next level of care and addresses the needs of patients who have experienced a critical event or diagnosis that requires management strategies to optimize health outcomes along the care continuum.
