Requirements

  • Licensed as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the State of Oklahoma
  • Experience in ambulatory care preferred
  • Demonstrated ability to lead in a team-based environment
  • Strong interest in working within an interdisciplinary health team and with a culturally diverse patient population
  • Excellent communication, organization, and time management skills
  • Familiarity with emerging professional and technical knowledge
  • Effective problem-solving skills and ability to multi-task
  • Compassionate with strong teamwork and advocacy abilities

Responsibilities

  • Promote a positive team environment, aligned with the mission of the health center
  • Track referrals and help patients overcome barriers to accessing care
  • Provide basic health education and explain provider instructions, procedures, and referral needs
  • Support preventative care efforts and help patients find affordable services
  • Assist uninsured patients with identifying and applying for available programs and services
  • Help patients complete applications for patient assistance programs
  • Advocate for patients by supporting informed decision-making about care and treatment options
  • Work closely with the medical team throughout the case management process
  • Engage with patients regularly to monitor progress and ensure patient satisfaction
  • Accurately document case information and complete all required forms
  • Promote cost-effective care and high-quality outcomes
  • Assess and address motivational and psychosocial needs
  • Follow all professional protocols, rules, and regulations
  • Recognize that this role evolves as patient needs change
  • Support clinical staff as needed by responding to nurse line calls, giving nursing advice, and performing other nursing duties as assigned
