About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency in Arizona, providing residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is a non-profit provider of substance use and behavioral health programs, utilizing cutting-edge, evidence-based models. The Case Manager delivers case management services in community and/or office settings to patients and their families. This role involves assisting patients and their care teams with planning, connecting, outreach, advocacy, coordination, and monitoring of needs and progress. Case Managers help patients access health services, financial aid, housing, employment, education, social services, and natural supports. Key responsibilities include assessing patient needs, developing and monitoring individualized treatment plans, and accurately documenting services. The Case Manager is expected to embrace the recovery philosophy, promote a safe recovery environment, and implement advanced techniques and interventions for high-risk individuals within their program.

Requirements

  • Highschool diploma or GED required
  • 1-3 years of experience working with the GMH/SU population required
  • 1-3 years of experience in behavioral health position providing outreach and engagement activities required
  • Valid ID required
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
  • Arizona Fingerprint Clearance card

Nice To Haves

  • Associates Degree or higher in a field related to behavioral health preferred
  • 1-3 years of experience in medical setting preferred
  • Minimum of six months of recovery from substance use and/or mental health disorders preferred
  • Current AZ Driver's License (valid and in good standing preferred)
  • Clean 39-month driving record (MVR) preferred

Responsibilities

  • Provides case management services in either the community and/or office setting to patients and their families
  • Assists the patient and care team in planning, connecting, outreaching and advocating, coordination and monitoring the patient's needs and progress
  • Assists patients in gaining access to health services, financial assistance, housing, employment, education, social services and natural supports
  • Assesses patients needs
  • Develop and monitor individualized treatment plans
  • Document services provided in an accurate and timely manner
  • Embraces the recovery philosophy and promotes a safe recovery environment for clients
  • Responsible for implementing advanced techniques and interventions to serve the high-risk individuals in their associated program
