The Case Manager is a non-exempt position responsible for coordinating family reunification services with family and/or qualified sponsors and assessing the needs of each unaccompanied child (UC) in care. Ensure trauma-informed theory and brief therapeutic techniques are used to work with unaccompanied children entering the United States Complete intake interview upon admission and promptly identify needs; Prepare assessments and any related social and medical history report or forms as they pertain to the client's needs and case record. Establishes a trusting relationship with the client. Identify, locate, and make contact with the child's sponsor within the United States Identify services needed and make referrals accordingly. Coordinate the service planning process. Ensure the concise and timely completion of all paperwork and documentation, including, but not limited to, administrative, clinical, and caseload requirements. Safeguard the human and legal rights of the clients. Coordination of discharge planning, including transporting unaccompanied minors to their destination within the United States, and coordination of post-release service Perform all other duties as assigned.
