SUMMARY Under supervision of the Program Director, the Case Manager is responsible for developing, implementing, and monitoring Life Plans for the people we support in one of our residential settings. This role ensures that services align with regulatory requirements, agency standards, and the personal goals of each individual. The Case Manager provides advocacy, coordination of services, and family engagement, while promoting dignity, independence, and quality of life for the people we support.
