The Case Manager at the RESET Center provides person-centered care coordination, engagement, and discharge planning to individuals utilizing the sobering program. Working from a harm-reduction and trauma-informed approach, the CM supports participants in stabilizing safely, explores immediate needs, and connects them to ongoing services in the community. This role conducts brief screenings and assessments, engages in crisis de-escalation when needed, and collaborates closely with community partners—including onsite Sheriff’s staff, local law enforcement, emergency departments, and outpatient providers—to facilitate rapid discharge and seamless linkage to care. The CM ensures participants leave the program with clear next steps and access to resources that promote health, recovery, and stability.
